Raised Inflammatory Markers Explained

It is a scenario many patients find unsettling: you receive your blood test results, perhaps via an app or a phone call from the surgery, and you see a result highlighted in red. Next to it, there are terms like CRP, ESR, or Plasma Viscosity, with a note stating they are "raised" or "elevated."

Naturally, the first instinct for many is to look for answers online. However, searching for "raised inflammatory markers" often leads to a list of serious conditions that can cause unnecessary alarm. In reality, these markers are some of the most common findings in clinical medicine, and a raised result is rarely a diagnosis in itself.

In this article, we will break down what these markers actually are, why your body produces them, and how a consultant physician interprets these numbers to create a sensible plan for your health.

What Exactly Are Inflammatory Markers?

Inflammation is the body’s natural response to "insult" or injury. Whether it is a bacterial infection, a sprained ankle, or a long-term condition like rheumatoid arthritis, your immune system releases proteins into the bloodstream to help coordinate a defense and repair process.

When we talk about "inflammatory markers," we are usually referring to three specific blood tests used frequently in the UK:

1. C-Reactive Protein (CRP)

CRP is a protein produced by the liver. It is an "acute-phase reactant," meaning it levels rise very quickly, often within hours, of an infection or significant inflammation starting. Because it rises and falls rapidly, it is an excellent tool for seeing how a patient is responding to treatment, such as antibiotics for a chest infection.

2. Erythrocyte Sedimentation Rate (ESR)

The ESR is a slightly more "old-fashioned" but still very useful test. It measures how quickly red blood cells sink to the bottom of a test tube. When there is inflammation, certain proteins (like fibrinogen) make the red cells clump together, causing them to fall faster. Unlike CRP, the ESR changes slowly and can stay raised for weeks even after an illness has passed.

3. Plasma Viscosity (PV)

Similar to the ESR, Plasma Viscosity measures the "thickness" of the liquid part of your blood. It is less affected by things like anaemia than the ESR is, making it a very reliable marker in certain diagnostic pathways, particularly when looking for complex inflammatory conditions.

Why Is My Result High? Common Causes

If you have a raised marker, the most important thing to remember is that these tests are non-specific. They tell us that inflammation is present somewhere, but they don't tell us where or why.

The causes can be broadly grouped into several categories:

1. Infection

This is the most common reason for a sudden, high rise in CRP. Whether it is a urinary tract infection (UTI), a chest infection, or even a severe dental abscess, the body’s immune response will push these markers up. Once the infection clears, the markers usually return to normal.

2. Physical Injury or Surgery

Any significant trauma to the body, a broken bone, a major burn, or a recent surgical procedure, will trigger an inflammatory response as the body begins the healing process.

3. Chronic Inflammatory Conditions

Conditions such as Rheumatoid Arthritis, Polymyalgia Rheumatica (PMR), or Inflammatory Bowel Disease (Crohn’s or Colitis) are characterised by long-term inflammation. In these cases, we use markers not just for diagnosis, but to monitor how well a patient's medication is working.

4. Other Medical "Insults"

Even a recent heart attack or a blood clot (such as a pulmonary embolism) can cause a rise in inflammatory markers as the body reacts to tissue damage.

When a Raised Result Isn't "Bad"

One of the reasons we caution against self-diagnosis is that many "normal" factors can cause these markers to sit slightly outside the laboratory's reference range.

  • Age and Sex: The ESR naturally increases as we get older, and it tends to be slightly higher in women than in men.

  • Weight: Being overweight or having obesity can lead to a state of low-grade, chronic inflammation, which often keeps the CRP mildly elevated (often between 5 and 15 mg/L) without any active "illness" being present.

  • Pregnancy: It is very common for the ESR to rise significantly during pregnancy due to changes in blood proteins.

  • Anaemia: If you are anaemic, your red blood cells can fall faster in the ESR test, giving a "false" impression of inflammation when the issue is actually iron levels or another form of anaemia.

The Consultant’s Approach: Treating the Patient, Not the Paper

When a patient comes to see me with a "raised marker," the first thing we do is move away from the computer screen and talk about the person. A blood test result in isolation is just a number; it only gains meaning when combined with a clinical history and a physical examination.

We look for "Red Flags" or accompanying symptoms that might point to a specific cause:

  • Unexplained weight loss or night sweats

  • Persistent fevers or rigors (shaking chills)

  • New or worsening joint pain and morning stiffness

  • Persistent changes in bowel habits

  • New lumps or bumps

If a patient feels entirely well and has a very mild elevation (for example, a CRP of 8 where the "normal" is 5), the most sensible and evidence-based plan is often to wait 4–6 weeks and repeat the test. Many of these mild rises are due to minor viral infections that the body has already dealt with.

However, if the markers are significantly high, for instance, an ESR over 100, this is a clear signal that the body is under significant stress, and we will initiate a much more thorough investigation, looking at everything from autoimmune markers to targeted imaging.

Why Specialist Review Matters

Navigating diagnostic uncertainty can be exhausting. Many patients find themselves in a loop of repeated GP appointments where tests are "monitored" but never fully explained or resolved.

A consultant-led assessment allows for "joined-up thinking." Rather than looking at the CRP in isolation, we look at the whole internal environment. We consider whether your symptoms, though perhaps vague, fit a pattern of a specific rheumatological or internal medical condition that might be missed in a standard ten-minute consultation.
